Please download the application form from the link below. Please note that if you have already completed an application form there is no need to re-send this or complete another form unless any of your details have changed.
Please note that the November and December sittings are now full. The Exams Co-ordinator will be in contact with all candidates to confirm places in due course.
Please note that a new Withdrawal and Extenuating Circumstances Policy is in place. Please ensure you read this before submitting your application form.
Before applying for the Final Membership Examination (OSPHE) candidates must have passed the UK's Diploma & Part I or Diplomate Examination (DFPH) or have been exempted from it, e.g. obtained reciprocal recognition of the Diploma & Part I Membership Examination held by the Irish Faculty of Public Health Medicine.
General advice is that the OSPHE should be taken and passed with at least two years (wte) of training remaining.
Trainees working part time should discuss their individual circumstances with regard to the timing of examinations with their training programme director.
