Significant events

A significant event is any unintended or unexpected event, which could or did lead to significant harm. FPH considers significant events to include anything which could threaten population safety or the reputation of your organisation. This includes incidents which did not cause harm but could have done, or where the event should have been prevented.

You should ensure that you are familiar with your organisation's local processes and agreed thresholds for recording incidents.

It is not the appraiser's role to conduct investigations into serious events.

If there has been no direct involvement in such incidents since your last appraisal, a self-declaration to that effect should be presented at your annual appraisal.

To help you reflect on any significant event, you may wish to useĀ this template.
