Diplomate Examination Application Form

Please may we bring your attention to the following points:

Reasonable Adjustments
The policy for applications for reasonable adjustments has recently been updated. Please ensure you have read the latest guidance in the above document before applying.

Each request for an adjustment to an exam must be made at the time of application and before the application closing date deadline.

Requests for adjustments can only be considered if the appropriate evidence from a medical professional or educational psychologist and supporting letter from a Training Programme Director/employer is submitted with the request.

To ensure the process is fair for all candidates, the Faculty requires all the necessary evidence to be supplied and submitted with the application for each exam and exam attempt. An adjustment cannot be considered without it and late documentation will not be considered. Please note that there is a fee associated with withdrawing if you wish to delay your attempt due to a condition that is currently being assessed.

The only exception to this is for candidates with long-term conditions who wish to apply for the same adjustment (that has previously been approved by FPH) at subsequent sittings of the same exam. Please see the Adjustment Policy for further details.

Payment and confirmation of booking
Places for the exam can only be confirmed once payment has been received. A link will be sent to you on submission of your application form (link below)

Our preferred method of payment is online by debit or credit card. FPH accept Visa or Mastercard.

Alternatively you may pay via  BACS transfer: Account No 36191159 Sort Code: 60-09-15.
If making payment via BACS transfer, please include 'DIP [Initial and Surname]' in the payment reference.

Please make payment within 48 hours of submission of your form to guarantee your place on the date you have selected. If your payment has not been received within 48 hours it may not be possible to hold your place.  As such, we strongly advise you to make payment on the date you submit your form as spaces are limited and guaranteed on a first come, first served basis. Once payment has been received you will be sent an email confirming your place. Please note that it will normally take several days to check and process entries. Receipt of all applications will be acknowledged.

Please note that if your employer is paying for your exam fee, payment will still need to be made before the application closing date. Purchase orders can take a few months to process so this may need to be arranged in advance of the application window opening in order to ensure payment is received in time. If payment is not received in time, the candidate will need to make payment in the interim to be able to sit the exam.

Withdrawing from the exam and Extenuating circumstances

The guidance for applications for withdrawals and extenuating circumstances changed in November 2019 and was updated in September 2020. Please ensure you have read the latest guidance in these documents before applying.
